- A certified copy of your birth certificate with a certified Czech translation.
- A medical certificate stating that you are in excellent health, no older than two months, with an official English or Czech translation.
- Legalized copies of your degrees, diplomas, or certificates translated into Czech by a sworn translator/interpreter listed on the Czech Republic’s list of experts and interpreters.
- Legal copy of diploma supplement or complete schedule of lectures and exams, including their scope and grades (numbers), translated into Czech.
Note: Applicants for doctoral programs must submit the following additional materials.
- A summary of your publishing activity and history.
- A summary of the specialized topics you desire to concentrate on during your doctoral studies.
- Your thoughts on the thesis’s concentration (3 to 5 pages in length).
- Request two recommendation letters from instructors at an academic institution.
